Facility Rental Online Waiver Form

Required

All people using the gymnasium will be required to sign a waiver form as specified by Albert College prior to use of the gymnasium. The group organizer is responsible to have waivers completed prior to use of the facilities.

Completed online waivers will automatically be submitted to Albert College. Please forward a copy of your confirmation email to your event organizer.

Athletics Facilities Care Commitment

Footwear: Only indoor, non-marking footwear is allowed in the gym. Outdoor shoes or any footwear that may leave marks are strictly prohibited.
Equipment Protection: All equipment must have proper protection to prevent injuries and damage to surfaces. Please report any wear and tear of equipment or facilities that could cause damage by contacting us at reception@albertcollege.ca.
Property Use of Equipment: Equipment should only be used for its intended purpose. For instance, placing benches on their sides to create barriers to corners is not allowed, as it can damage the floors.

I acknowledge that I understand and agree to the Athletics Facilities Care Commitment as outlined above.

Terms of Servicerequired

Unless I advise you otherwise in advance in writing, I approve of my participation in this activity, acknowledged that such participation involves risks and hazards incidental thereto, all of which are expressly assumed, and do hereby waive, release and absolve and agree to indemnify and save harmless Albert College and its Directors and employees of and from any and all liability arising therefrom, except such as a consequence of its or their willful neglect or default. It is understood and agreed that Albert College is entitled to require the withdrawal of my participation at any time if it is determined that no other sanction is appropriate in the circumstances of inappropriate conduct or disrespect towards staff and students.
